Beneficiary changes
Beneficiary forms for changes can be requested through the Savings Plan Information Line at 1-800-892-2445 or the FirstEnergy Pavings Plan Web site at http://fesavingsplan.csplans.com where you can download these forms. Forms should be returned to the Human Resources Service Center, 2800 Pottsville Pike, Reading, Pennsylviania 19605.
